*5. Physical examination record for foreigner

Filled in English. The medical examinations must cover all the items listed in the Foreigner Physical Examination Form. Incomplete records or those without the signature of the attending physician, official stamp of the hospital or a sealed photograph of the applicants are invalid. The medical examination results will be valid for 6 months. All applicants are kindly requested to take this factor into consideration while determining the time to take the medical examination.

*6. High school graduate certificate

Graduation certificate in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.

*7. High school academic transcript

Academic transcript in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.

*8. Passport photo

A recent passport-sized photo of the applicant

*9. Photocopy of valid passport

With name, passport number & expiration date, and photo included

*10. CSCA Test Transcript(s)

-The test covers Chinese (Humanities and STEM Chinese), Mathematics, Physics, and Chemistry. Mathematics is required for all programs, while other subjects depend on the university’s specific requirements.

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
